KINDERGARTEN
REGISTRATION

Will your child be five on or before September 1, 2008?

WHAT DO I BRING WITH ME TO REGISTER?   

                Proof of Birth: Acceptable documents include:

·      Birth certificate

·      Certificate of baptism

·      An insurance policy on the child’s life

·      A bona fide religious record of the child’s birth

·      A passport or visa

·      A transcript of record of age in the child’s previous school record

·      If none of the above can be produced, an affidavit of age sworn to by the parent, accompanied by a certificate of age signed by a public health officer or by a public school physician, which states that the health officer or physician has examined the child and believes that the age as stated in the affidavit is correct.


A homeless child shall be given 30 days to meet the above requirements.

¨          Results of a Florida physical (school-entry health exam) performed within one year of the date of enrollment.

¨          Proof of address. Acceptable documents include:                                         

·      Jacksonville Electric Authority (JEA) bill,

·      Credit card statement,

·      Mortgage agreement,

·      Lease agreement (from a company, not a private individual) or

·      Piece of mail forwarded to the current address and containing a yellow U.S. Post Office forwarding address sticker.

¨          Proof of all required immunizations (Florida certification of immunization, DH Form 690) or an exemption.

IMMUNIZATION REQUIREMENTS 

* Florida law requires that students must have on file Florida Certification of Immunization (DH Form 680) or have an exemption on file at their school.

To meet the requirements for Florida certification of immunization, a child must have the following:

·      Five doses of the DTP (diphtheria, tetanus, pertussis) vaccine

·      Four doses of IPV (polio) vaccine

·      Two  MMR (measles, mumps, rubella) vaccine

·      HIB (haemophilus influenza type b) vaccination

·      Hepatitis B series ( 3 )

·      Varicella (2chicken pox) vaccine, or documented history of the disease

 

¨          The required immunizations are available at the Duval County Health Department's Immunization Center at 5222 North Pearl Street, (359-3814).

 

¨          Under Florida law, students who do not have the proper Florida immunization documents on file at their school will not be able to attend school until documents are provided.
